Insurance & Billing Info:
At the time of your visit, you will be expected to pay for the services provided if you participate with an insurance plan we are not on. As a courtesy, we will be glad to submit your claims to your insurance company if you so wish.
Our 2021 annual administration fee is $500. It is payable on your first visit of the calendar year. If you have specific questions regarding your administration fee, please contact the office manager directly.
We accept Visa, Master Card, American Express, checks, and cash.
Our office policy is to accept only regular Medicare and supplement policies. We do not accept Medicare replacement or Medicare advantage plans.
**We are not accepting new Medicare patients into the practice at this time unless referred by an established family member or referring specialist.
We are contracted with Arizona Foundation for Medical Care, Cigna PPO/Open Access Plus, GWH-Cigna, and CCN/First Health insurance networks. Your insurance is a contract between you and your insurance company. You are responsible for all bills regardless of the type of insurance coverage you may have. If no payment is received from your carrier within 90 days you will be responsible for the charges incurred and billed directly. Please be sure to advise us of any address, telephone, or insurance changes. It is very important that our staff has your correct information so they can bill appropriately and in a timely manner.
